My Complaint letter to Linda Hann, PALS and copy for Paul Mears.
Dated 3 October 2014 and before Paul Mears, Chief Executive at Yeovil Hospital at that Meeting on the 11 December 2014.
I am writing to you today, as it has been now 6 Months since I first came into A & E so very ill on Thursday 3 April 2014.
I have in al that time done all possible in trying to make contact within letters/phone calls and done my best in tying to sort this matter out since leaving Yeovil Hospital on Saturday 31 May 2014.
I am now writing my full complaint to you, as I have spoken to you before on the telephone and I know that you are aware of this situation that should have been sorted out long ago and can’t go on any longer now.
I now want everything looked into and I expect a meeting with you and this may even take more then just that one meeting, as I won’t be given up on this and such torment and suffering over my health and life.
Every Patient has a Right to be told what is wrong with them and every Patient has a Right to be treated with care and respect and such standards should not fail so badly within Yeovil Hospital.
From leaving Yeovil Hospital all I have suffered is torment and pain for Months on end and nothing sorted out and it’s been like a nightmare. I rang Mr. Fosters Secretary over and over again and I rang Urology Department so many times. I have been worried and upset and couldn’t get any answers or sense over all this with my health.
I will write starting back from Thursday 3 April until Saturday 31 May 2014 and then I will go on to write about since leaving Hospital in June, July and August 2014.
1. Thursday 3 April 2014 – First visit into A & E
2. Wednesday 9 April 2014 – Second visit into A & E
3. Wednesday 9 April until Friday 18 April 2014 ( Intensive Care / 9B ) 10 days

Leaving Yeovil Hospital on Friday 18 April 2014.
4. Thursday 15 May 2014 – Outpatients for Colonscopy with Dr. Stephen Core
5. Wednesday 28 May 2014 – Outpatients, pre-op check with Nurse, Jeff Parkinson
6. Friday 30 May 2014 – Operation with Mr. Foster, Urology, Kingston Wing
Leaving Yeovil Hospital on Saturday 31 May 2014.
The very start, My first visit into A & E, very sick and unwell. Told it was something I had eaten and it would take a few days before I would start to feel better and so I needed to go home and see it out.
Please could you now look into this and blood tests, x-ray taken and medication given.
Please look into results and why it was decided to not keep me in and send me home on that day.
Then my Complaint moves on from there onwards and it is all very clear as to all I am asking these Mangers to look into and also we spoke about this within conversation on the 11 December 2014.
